Therapy is an investment in your well-being that we take seriously and want to provide you with the best therapeutic care. At Elevate Mental Wellness we accept one insurance, HealthChoice. We made this decision due to a number of factors including privacy, confidentiality, control, and limitation issues.
When choosing to use your insurance, you may want to consider the following important information:
When using your insurance, we may need to disclose clinical documents to justify the need to continue which does not keep what you share 100% confidential. There are additional limits to confidentiality that will be explained at your intake appointment which you will be asked to review and sign in your consent form.
When using your insurance, they dictate what you can get approved for, how much they are willing to pay, and how often you can receive these services. As the insurance member, the quality of care is limited to what or whom the insurance can offer. When you choose to pay privately, you have control over which service you need, plus the amount of necessary sessions at a rate that works best for you.
Many services covered under insurance have a cap on how often services can be rendered and received.
Insurance companies frequently audit files and deny claims and services as they deem fit. By not using insurance, you have control over who has access to your records.
In order to have insurance reimbursed for services, there needs to be a qualifying diagnosis. Without a diagnosis, the therapist will not be able to submit a bill for insurance payment reimbursement. Many people seek therapy for personal growth and normal life stressors, which do not meet the requirements for a mental health diagnosis. Some people also choose not to have a diagnosis on file. This is your right and something you get to decide by not using insurance.
Private-Pay Session Fees:
Individual therapy sessions (50-55 minutes): self-pay $120
Assessment/Intake session (60-75 minutes): self-pay $140
Family Therapy Sessions (50-55 minutes): self-pay $130
Assessment/Intake session for family (60-75 minutes): self-pay $160
Sliding Scale Fees:
A reduced fee based on household income may be available if needed. Limited sliding scale fees are available for individuals demonstrating significant financial need and meet specific qualifications determined by the therapist. Sliding scale fees are subject to increase at any time, and the discount will be terminated if the client is not consistent with appointments.
We accept all major credit and debit cards.
This includes (FSA and HSA debit cards) for services.
Elevate Mental Wellness is private-pay based (or known as self-pay) and accepts HealthChoice insurance only.
All clients will be required to put a valid card on file when scheduling their initial intake appointment. If payment is not secured your appointment will be cancelled and will need to be rescheduled for another date.
If you are a no-show or cancel less than 24hrs before your scheduled appointment you will be charged the full fee for your session. You can call and/or text 405-673-4002 or send an email to email@example.com within the time frame noted.
