Therapy Services:

Individual therapy sessions are designed to collaboratively work with you to develop a plan that meets your needs. Individual therapy is one type of psychotherapy in which a trained professional helps a single person work through personal issues they have been facing. It is an effective treatment for a variety of emotional difficulties, mental health challenges, and mental illnesses.

Rates and Payment Information:

Self-Pay Session Fees: 

Initial Assessment/Intake session (60-75 minutes): self-pay $140 

Initial Assessment/Intake session for family (60-75 minutes): self-pay $160

Individual therapy sessions (50 minutes): self-pay $120 

Family Therapy Sessions (50 minutes): self-pay $130

Sliding Scale Fees:

A reduced fee based on household income may be available if needed. Limited sliding scale fees are available for individuals demonstrating significant financial need and meet specific qualifications determined by the therapist. Sliding scale fees are subject to increase at any time, and the discount will be terminated if the client is not consistent with appointments.

Payments:

We accept all major credit and debit cards.

This includes (FSA and HSA debit cards) for services. 

Insurance Plans Accepted:

Elevate Mental Wellness is self-pay based and accepts Health Choice only at this time. 

If you're looking to use insurance we don't accept, please send us an email to discuss alternative options.

Please Note: 

All clients will be required to put a valid card on file when scheduling their initial intake appointment. If payment is not secured your appointment will be cancelled and will need to be rescheduled for another date.

Cancellation Policy: 

If you are a no show or cancel less than 24hrs before your scheduled appointment you will be charged the full fee for your session. You can call and/or text 405-673-4002 or send an email to info@elevatementalwellnessllc.com within the time frame noted.
